Phase 1 is designed to engage families earlier in their decision-making journey—before discharge planning or urgent care needs arise. By offering valuable planning tools and education around recovery support at home, we build trust, generate qualified leads, and create stronger consultation conversations. This phase lays the foundation for Phases 2 and 3, which will support families during and after discharge.
